Most teams treat bounce rate as a campaign-level metric: something you glance at after a send, shrug about if it's under 5%, and move on. That's the wrong frame. Bounce rate is a reputation input, not a campaign output — and reputation compounds. A list that bounces at 8% today doesn't just cost you 8% of that send; it costs you inbox placement on the other 92% for months afterward, across every domain and IP tied to that sending infrastructure. In healthcare data specifically, where HCP contact info decays faster than almost any other B2B vertical, this problem is structural, not incidental.
Here's a working checklist for catching it before it becomes a domain-level liability.
Before You Buy or Import a List
- Ask for the verification date, not the verification claim. "Verified" means nothing without a timestamp. A list verified eight months ago is functionally unverified for HCP data — NPI records, practice affiliations, and hospital email formats shift constantly.
- Get field-level match rates, not just syntax checks. Syntax validation ("this looks like an email") is not the same as confirming the mailbox exists and is tied to the named provider. Ask the vendor to break out how they distinguish the two.
- Run a sample against your own suppression and bounce history first. Before uploading a new list wholesale, cross-check 500–1,000 records against records you've already flagged as bad. If overlap is high, the source is recycling stale data.
- Confirm how the data was sourced. Opt-in, compiled from public directories, or scraped from hospital system staff pages each carry different decay and bounce risk. You should know which one you're buying.
- Check domain concentration in the list. A list dominated by a handful of large hospital system domains means your reputation on those domains is shared with every other sender hitting the same addresses — including bad actors you have no visibility into.
During Onboarding and the First Sends
- Warm up new domains and IPs deliberately. Sending the full volume on day one to a fresh domain is the single fastest way to trigger spam filtering, regardless of list quality. Ramp over 2–3 weeks.
- Segment the first sends by data source and age. Don't blend a freshly acquired list with your existing house file on the first campaign — you won't be able to tell which source is driving the bounce rate if something goes wrong.
- Set an automatic pause threshold. Decide in advance — 2%, 3%, whatever your platform and volume can tolerate — and configure the system to halt sending, not just alert you, when hard bounces cross it mid-send.
- Separate hard bounces from soft bounces in your monitoring, not just your reporting. A spike in hard bounces during onboarding tells you the list itself is the problem. A spike in soft bounces might mean a temporary block or a mailbox provider throttling your new domain — different fix entirely.
- Account for enterprise mail security silently swallowing sends. Many hospital and health system domains route through Proofpoint, Mimecast, or similar gateways that reject or quarantine mail without generating a clean bounce code. Your bounce rate may look fine while your actual delivery to that segment is near zero — check engagement data, not just bounce logs, to catch this.
After the First Delivery Wave
- Pull and categorize bounce codes, don't just count them. "Mailbox not found," "domain does not exist," and "blocked by recipient policy" require different responses. Lumping them into one bounce percentage hides which problem you actually have.
- Remove hard bounces same-day, not on your next scheduled list cleanup. Every additional day a dead address sits in an active send list is another data point telling mailbox providers your list hygiene is poor.
- Re-verify soft bounces before retrying, not immediately after. Retrying a soft-bounced address within minutes often just repeats the same rejection. Wait 24–72 hours, re-check status, then resend.
- Check your standing with Google Postmaster Tools and Microsoft SNDS. These are free, and most teams never look at them until reputation has already visibly degraded. Domain and IP reputation scores move before your open rates do — that's your early warning system.
- Look at inbox placement, not delivery confirmation. "Delivered" can mean spam folder. If you have access to seed list or placement testing, use it after any send where bounce rate ticked up — placement often degrades before bounce rate spikes, and recovers slower.
Ongoing Governance, Not One-Time Cleanup
- Set a refresh cadence tied to actual HCP turnover, not an arbitrary calendar. Specialty, practice size, and region all affect how fast this data goes stale — a static "review every six months" policy will be wrong for at least part of your file.
- Track reputation metrics on a rolling basis, across campaigns, not per-send. A single bad send is noise. A downward trend across four consecutive sends is signal — and by the time it shows up in your open rates, the underlying reputation damage is already weeks old.
- Build a suppression path for chronic non-engagers, separate from hard bounces. Addresses that are technically deliverable but never open or click are a slower-burning version of the same problem — mailbox providers use engagement, not just bounce status, to score you.
- Isolate risk by domain when testing new or lower-confidence data. Don't run an unproven list through the same domain carrying your core revenue campaigns. If it underperforms, you've now put your entire program's reputation at risk to test one list.
- Know your escalation levers before you need them. When reputation metrics dip, decide in advance whether the response is throttling volume, rotating IP pools, pausing the affected segment, or re-warming — figuring this out mid-crisis costs you more time than the crisis itself.
The teams that treat this well aren't the ones with the cleanest lists — they're the ones with the tightest feedback loop between data quality and sending behavior.
